  Baptismal Certificates
  
  Children enrolling for the first time must provide a copy of their Baptismal Certificate, even if they were baptized at Resurrection Parish. Please submit a copy to the parish office or email it to [email protected].
  
  Parish Registration
  
  New families are required to register with the parish when enrolling in the Faith Formation program. Parish registration forms are available on our parish website or at the parish office. Your family’s unique journey matters to us. To ensure we serve you in the best way possible, we will be in touch to schedule a brief 'get to know you' meeting with our Faith Formation team before the school year starts. We look forward to welcoming you!
  
  Family Events (All Programs)
  
  All families in grades 1-8 attend seasonal family events during Advent/Christmas and Lent/Easter. Families select one event per season that best fits their schedule. These gatherings help families grow in faith together and recognize Jesus more fully in everyday life.
  
  Faith Formation Agreement
  
  I agree to partner with Resurrection Parish for my family's faith formation. I understand that this includes regular attendance at Mass and class as well as completion of “Faith at Home” assignments for all families enrolled in the Pathways of Faith program. Large gaps in attendance may result in repeating a grade or a delay in receiving the sacraments. Program details on the Parish website can be found here.

  Faith Formation Payment
  
  The fee schedule below applies to all Faith Formation programs. Please select the checkbox(es) corresponding to the program(s) for which each child is being registered. After selecting all applicable program fees, click “Proceed to Payment.”
  
  Please note: We want every family to be able to participate in Faith Formation. If registration fees present a financial hardship, please contact Father Yojan. All conversations will be handled with care and confidentiality.
